Benefit Summary

We know that quality benefits and the opportunity to choose are important to you and your family. With that in mind, The City of Apopka maintains a high-quality benefit program. The following benefits are in effect for the plan year October 1, 2023 through September 30, 2024.

Employee Health and Wellness Center - My Health Onsite! (Paid by City)

Employees and their dependents (8 years of age and older) covered by the City’s health insurance plan have access to no cost general medical care at the medical facility provided by the City. There are no charges for examinations, no charge for prescriptions dispensed, no charge for lab work, and any other services available on-site. The facility also offers access to wellness programs, including weight-loss programs, tobacco cessation programs and other wellness initiatives.

Health Insurance – Blue Cross Blue Shield of Florida

Employees have a choice between four plans. BlueOptions 03559 PPO Plan (employee premium paid 100% by City), BlueChoice 0727 PPO Plan, BlueCare 55 HMO Plan and the BlueCare 05180/05181 PPO High Deductible Plan (HDHP) (employee premium paid 100% by City), all plans have benefit-rich designs. Please see plan documents for more information.

The PPO plans allows for self-directed care. You may choose which doctor(s) you would prefer and you may go to specialists without referral. The plan includes an out-of-network option, however, BCBS maintains a very large provider network and out-of-network provisions carry much higher costs for the member. Regardless of network, there is world-wide emergency coverage.

The HMO provides for self-directed care within the BlueCare network. This plan does not require a deductible or co-insurance. There is no out-of-network coverage provision, you must remain in-network. Regardless of network, there is world-wide emergency coverage.

Group Life Insurance – CIGNA Group Life (employee basic premium paid by City)

A life benefit of $30,000 is provided for each employee at the City’s expense. Employees have the opportunity at the time of hire to purchase extra life insurance (maximum of $350,000, not to exceed 5 times base annual pay) with guaranteed issue of purchases up to $200,000, at initial eligibility, regardless of health. If coverage is applied for at a later date (or if an upgrade in coverage is requested at a later date) “Evidence of Insurability” must be provided, including health questions.

Dependent life policies are also available. A policy providing a $15,000 benefit for Spouse is available, as well as a $5,000 per child policy (age 6 months to 19 years, as defined in the policy; age 14 days to 6 months provides a $500 benefit). The dependent policies cannot be increased and are purchased separately. Should you choose to enroll your spouse at a time other than an initial enrollment period, “Evidence of Insurability” must be provided.

Employee Assistance Program (EAP) – Resources for Living (paid by City)

EAP benefit providing multiple types of counseling, including work/life balance, legal, financial, etc. Available to all employees and their households. That includes dependent children up to age 30, whether or not they live at home. Coverage provided at the City's expense – 5 free, confidential counseling sessions per issue each year, additional sessions are at the employee's expense, if charges apply.

Dental Insurance – Florida Combined Life (paid by employee):

Employees have a choice between two plans. Both plans are PPOs and have “open access” within the network, plus you have the option to go outside of the network. The low option plan, BlueDental Choice Plan, is a good basic dental plan. The high option plan, BlueDental Choice Plus Plan, might be a better choice if you expect to have heavier utilization of the dental plan.

Voluntary Short-Term Disability (STD) Insurance- New York Life (paid by employee)

Benefits begin after a person is disabled for: Accident (1 day) and Sickness (7 days). This benefit is 60% of your weekly salary (up to a maximum of $1500 per week). The maximum benefits period for Accident is 26 weeks and for Sickness is 25 weeks from the date of disability.

Long-Term Disability (LTD) Insurance – New York Life (paid by employee)

Benefits begin after a person is disabled for 180 days (six months). The benefit is 60% of your monthly salary (up to a maximum of $5,000 per month). The plan has a Two-Year “Own Occupation” benefit. This important provision means that for the first two years of disability, the policy will pay you benefits as long as you can’t work at the position for which you’ve been educated or trained. (Some LTD policies don’t pay any benefits unless you are completely disabled and unable to perform any gainful employment). Benefits are payable up to age 65. For those over 65, the benefit period will vary based on the age at which you first purchased coverage. (See LTD brochure).

Flexible Spending Account (paid by employee)

Each employee is allowed to make tax-sheltered contributions to a Flexible Spending Account which may be used to pay for qualified Medical Expenses. Your contribution is made on a Pre-Tax basis, with a maximum employee contribution which is set each year by the Internal Revenue Services (IRS).

It is very important to note Federal Regulations stipulate a “Use It or Lose It” provision for FSAs, unless the plan sponsor chooses to establish a “rollover” provision. The City has established a rollover provision which allows year-end balances IRS annual allowed $610 to “roll” to the following plan year (see Summary Plan Description). Any unused funds over $610 in your FSA account at the end of the Plan Year are forfeited. Your Plan Year runs from October 1st to September 30th.

Voluntary Supplemental Benefits –Colonial (paid by employee)

New hire’s will be given the opportunity at a separate time to select from the supplemental products available through Transamerica. In order to maintain your initial eligibility for guaranteed issue products, you must meet with a representative at the first available meeting following your date of hire (generally the next open-enrollment season).

These products include, but are not limited to, Accident/Disability; Cancer; Critical Illness; Hospital policies; and various life and long-term insurance choices. These products are available at the employee’s expense.

Employees are eligible for the above listed coverages on the 1st day of the month following 30 days of full-time employment.
